Qualifications of our Agency
Quality Healthcare Services is not only a Real Life Choices Provider, but also a qualified provider under the “Individualized Community Supports and Services” for developmentally disabled individuals under the administration of the Division of Developmental Disabilities, NJ Department of Human Services.
We provide our clients with personalized care. To do this, a nurse will first assess the client’s health care needs and will then work with the physician and/or family members in the development of an individualized care plan. This care plan will outline the set of services, schedule and level of care that the client will receive to preserve his or her health. You can request for an assessment schedule online.
